  Background
Blood culture is the most important test for diagnosis and treatment of bloodstream infection. Contamination in process of sampling and blood culture causes abuse of antimicrobial agents. So, we started with quality improvement activities for reducing blood contaminations and obtaining reliable results.

Methods
We established quality indicators about blood culture test (true positive rate, skin contamination rate, blood sampling volume, one set of blood culture ordered, delayed transportation rate, sample collecting ratio of femoral region) following the CLSI guideline (M47-A, 2007). Education were carried out for interns and nurses who are responsible for blood sampling and transportation of specimens. For monitoring the effect, data were analyzed monthly.

Results
During basement survey, the overall contamination rate was 4.0%. During quality improvement period, the average contamination rate was 1.9%. The rate of bottles below the suggested blood volume was 29.7% to 2.7~11.3%. Delayed transportation rate was decreased from 35.6% to 5.5~7.7%. Sample collected from femoral region rate was decreased from 41.5% to 22.0~31.0% and couldn’t attain the goal.

Conclusion
Results of activities of quality improvement provided us possibility of reliable changes such as decreased contamination rate, increased specimen volume, shortening specimen transportation time and decreased femoral region samples. We were relatively careless for quality improvement activities of blood cultures in 2011, and found increased bottles below the suggested blood volume, femoral region samples and contamination rate. Therefore, reinforcement of management would be able to cause more reliable results of blood cultures.
