  Background: Speed and accuracy of the blood culture test is important for diagnosis of blood stream infection. To this end, SCL as An automated blood culture system (ABC system) was introduced in SCL, a commercial laboratory. The impact on the diagnosis and treatment of bacteremia in small clinics was analyzed to verify the effectiveness of the blood culture tests done on an automated system.
Methods: Blood culture positivity, strain-specific positive rates, sensitivity, and detection rates in primary positive reports and final reports were compared between manual method and automated blood culture method to evaluated the reliability and efficiency of the ABC system
Results: The blood culture positive rates of Enterobacteriaceae, Gram-positive pathogens and fungus for the manual method versus automated method were 3.03% versus 3.7%, 2.14% versus 2.63%, 0.33% versus 0.82%, respectively. Likewise, the opportunistic pathogen detection rates were 5.50% versus 9.07% for the manual method versus automated method.
In the primary positive report, the detection rate of the blood culture was significantly higher in the automated method (18.1%) compared to the manual method (12.0%). In the final report, the isolation rate of the blood culture was 11.8% in the manual method and 17.1% in the automated methods, which confirms the higher sensitivity of the automated blood culture.
Conclusions: Despite being vulnerable to sample management and transport, and the result notification process, the ABC system showed better sensitivity (culture positive rate) and detection rate than the manual method. Thus, the ABC system is also considered to have great clinical utility in a commercial laboratory.
